• Foster Care Status Verification:

  • Court Reports/Case Plan

  • Foster Care Verification Letter: Obtained from the Office of the Foster Care Ombudsperson

• Transitional Independent Living Plan

  • Six-Month Certification (SOC 161): For extended foster care participation.

  • Mutual Agreement (SOC 162/163): For non-minor dependents (NMDs).

• Medi-Cal Card/Health Passport

• Education Records: Transcript or school contact information.
